For Referring Doctors

Doctors and staff can easily refer patients to our Alameda office by using our professional referral form. This allows us to understand the specific concerns and restorative status of the patient before their first visit.

Referring clinicians can indicate:

  • Specific clinical concerns (spacing, bite, crowding, etc.)
  • Status of restorative treatment.
  • Requests for specific follow-up (calls after exam or written reports).
  • Availability of recent panoramic radiographs.

Refer a Friend or Family Member

While many of our patients are referred by their family dentist, we also welcome referrals from our current and former patients. If you have a friend or family member who could benefit from an orthodontic screening, simply have them contact our office to schedule their initial consultation
